This service, subsidised by the Government Social Welfare Department, was launched in 2008. Operating in Hong Kong Island East, it provides comprehensive, high-quality and professional daytime, home-based and community care services. This allows frail elderly persons to receive continuous care in their familiar community, enabling them to lead a quality life in their later years and achieve the goal of "aging in place". Moreover, the service supports their families and caregivers, helping to relieve their caregiving stress.
Frail elderly persons aged 60 or above living in the community and who are assessed and recommended for Community Care Services or Residential Care Services under the Standardised Care Need Assessment Mechanism for Elderly Services
