About the Programme:
The Methodist Centre successfully bid for the Integrated Support Programme for Discharged Elderly (Home Support Team) at the Union Hospital and the Ruttonjee Hospital in the Eastern District, which started its service in October 2011. In addition, the programme also won the bid for the Integrated Support Program for Discharged Elderly (Home Support Team) at the Mary Hospital in the Western District, which officially commenced in April 2023. The program provides home-based, rehabilitation, and community support services to discharged elderly individuals, reducing the risk of emergency readmission. It also offers training and support services to caregivers, alleviating the pressure of caring for the elderly.
